Assessments

HCS Registered nurses are available to attend to your health care needs by preserving health and handling existing health problem that you may have. Our main focus is to assist you to maintain your independence, continue a safe and healthy way of life, as well as helping your love ones. We are open daily from 9am - 5pm.

Care Planning, Case Management and Community Resource Coordination

Initially, an assessment will be undertaken in order to gain an understanding of your current health condition and tackle how to address your concerns in a person-centred manner. Then, a support plan will be created as per the discussion with you and/or your loved ones which also incorporates the recommendations from other health care practitioners involved in your care. In the care plan, it will include a detailed service you require including the hours, timing and days which suits your schedule. After distinguishing the services needed, Horizon Community Services will arrange staffing for the services which may include meal preparation, medication administration, and personal hygiene care, domestic cleaning and gardening services. Horizon Community services Registered nurse works in close liaison with you and/or your love ones. For continuity of care, we will constantly review your support plan in a regular basis or as changes require ensuring we meet your health goal and live life at your best.

HCS Nursing Services includes:

• Continence assessment and management

• Diabetes management including insulin administration

• Catheter Care

• Wound Care

• Medication Assistance

• Tracheostomy Care

• Subcutaneous management

• PEG Feed

• Ventilator management.
