Transition to Practice

The Rural Team Based Care (RTBC) Initiative will bring together rural health professionals to work collaboratively to improve the health outcomes of patient with chronic and complex health conditions, particularly those at risk of hospitalisation within the next 12 months.
By forming a primary care collaborative, healthcare providers in the targeted rural community the local sector will develop strong inter-professional working relationships and work towards building a model of care that fosters enhanced multidisciplinary health care to their patients.
