Bamford Community Care

Our story began by like-minded Individuals coming together and wanting to create a positive change to human supports and services. Frustrated at a lack of frameworks and conceptual understanding of others, the management team have come together to create a service provider which promotes true Person-Centred Care. The Bamford Community Care management team are committed to seeing their human supports and services defined in terms of quality in the lives of people accessing our services. Bamford Community Care seeks to work with participants to obtain meaningful dignity and quality of life.

Our Services and Supports

Bamford Community Care provides the following support services for participants with a disability: 

o   Assistance in coordinating or managing life stages, transitions, and supports. 

o   Assistance with daily personal activities. 

o   Assistance with travel or transport arrangements. 

o   Community nursing care. 

o   Assistance with daily life tasks in a group or shared living arrangement. 

o   Household tasks.

o   Participation in community, social and civic activities. 

o   Development of daily living and life skills

NDIS Supports:

Accessible Transport

• This service provides for specialised transport services for a person to a school, educational facility, employment, or the community.

Assistance with Self Care

• This service provides a person with assistance with, or supervision of, personal tasks of daily life to develop skills of the participant to live as autonomously as possible.

Access Community

• This service enables a person to engage in community, social and recreational activities.

Assistance with Personal Domestic Activities

• This service provide a person with assistance with, or supervision of, personal tasks of daily life to develop skills of the participant to live as autonomously as possible.

Assistance with Household Tasks

• This service enables a person to maintain their home environment. This may involve undertaking essential household tasks that the participant is not able to undertake.

Support Coordination

• This service strengthens a person's ability to design and then build their supports with an emphasis on linking the broader systems of support across a complex service delivery environment.
